What Is the Independent Care Waiver Program (ICWP)?

The Independent Care Waiver Program (ICWP) is a Georgia Medicaid home and community-based waiver program designed to help eligible adults with significant physical disabilities or traumatic brain injuries receive services and supports in their homes and communities rather than institutional settings.

ICWP can provide access to a variety of approved services based on an individual’s eligibility, assessed needs, and plan of care.

Case management is an important part of helping participants navigate these services and coordinate the supports available to them.

Who Does ICWP Serve?

Georgia’s ICWP is designed for eligible adults with significant physical disabilities or traumatic brain injuries who meet the program’s requirements.

Eligibility is determined by the appropriate Georgia Medicaid agencies and may involve factors such as age, Medicaid eligibility, level-of-care requirements, medical and functional needs, and other program criteria.

Hooks Case Management does not determine Medicaid or ICWP eligibility, but our team can help participants and families better understand the case management process and their next steps.

What Does an ICWP Case Manager Do?

An ICWP case manager helps coordinate the services and resources a participant needs to live as independently as possible in the community.

Depending on the participant’s needs and approved services, case management may include:

Individualized Care Planning

Helping develop and maintain a person-centered plan based on the participant’s needs, goals, preferences, and approved services.

Service Coordination

Helping coordinate authorized providers, services, and supports included in the participant’s plan of care.

Monitoring & Follow-Up

Maintaining ongoing communication and monitoring services to help ensure the participant’s needs and goals continue to be addressed.

Community Resources

Helping participants identify appropriate community resources and additional supports that may be available.

Advocacy & Participant Support

Helping participants understand their services, communicate needs, and remain involved in decisions about their care.

ICWP and Activities of Daily Living (ADLs)

Many individuals begin researching Medicaid waiver programs because everyday activities have become difficult.

These are often referred to as Activities of Daily Living (ADLs) and may include:

  • Bathing

  • Dressing

  • Eating

  • Toileting

  • Mobility

  • Transferring

  • Other personal care needs

Difficulty completing ADLs may indicate that an individual needs additional services or support.

An ICWP case manager can help eligible participants coordinate authorized services based on their assessed needs and approved plan of care.

Physical Disability & Traumatic Brain Injury Support

ICWP specifically serves eligible individuals with significant physical disabilities and traumatic brain injuries.

Living with a physical disability or TBI can involve more than medical care. Individuals may need assistance coordinating services, navigating community resources, managing changes in their needs, and maintaining as much independence as possible.

Hooks Case Management provides person-centered case management focused on the individual—not simply a diagnosis.

Traditional & Enhanced ICWP Case Management

ICWP participants may receive different levels of case management based on their needs and program requirements.

Traditional Case Management

Traditional case management helps participants coordinate approved waiver services, maintain their plan of care, access resources, and monitor their ongoing needs.

Enhanced Case Management

Some participants with more complex needs may qualify for enhanced case management, which can involve additional coordination and monitoring.

The appropriate level of case management is determined according to Georgia Medicaid and ICWP requirements.
