Hooks Case Management Services, LLC provides person-centered case management for seniors and individuals with disabilities throughout Georgia. We help clients and families understand their options, coordinate essential services, connect with community resources, and receive continued support through every stage of care.
Led by Ms. Tracie Hooks, CEO and Owner, HCMS proudly provides statewide case-management support through the Independent Care Waiver Program and serves 10 Georgia counties through the Elderly and Disabled Waiver Program.
