Hooks Case Management Services, LLC provides compassionate, person-centered support to seniors and individuals with disabilities throughout Georgia. Led by Ms. Tracie Hooks, CEO and Owner, our team works closely with clients and families to assess their needs, create personalized care plans, connect them with valuable community resources, and coordinate the services they need to live with greater independence and confidence.
We proudly serve individuals through the Independent Care Waiver Program (ICWP) and the Elderly and Disabled Waiver Program (EDWP). At HCMS, every client is treated with dignity, respect, and genuine care.

SUPPORT THROUGH GEORGIA’S WAIVER PROGRAMS
Hooks Case Management Services helps individuals and families understand their options, coordinate essential services, and receive dependable support through Georgia’s ICWP and EDWP waiver programs.
INDEPENDENT CARE WAIVER PROGRAM (ICWP)
ICWP helps eligible individuals with disabilities receive services that support greater independence at home and in the community. HCMS provides statewide case-management support, connects clients with helpful resources, coordinates services, and monitors their care as their needs change.
ELDERLY & DISABLED WAIVER PROGRAM (EDWP)
EDWP helps eligible seniors and individuals with disabilities receive community-based services and support. HCMS provides case management in Bibb, Crawford, Peach, Twiggs, Houston, Monroe, Henry, Spalding, Upson, and Pulaski counties.
